#1 Statutory Declaration help!!

Hi guys, i'm wanting to send a Statutory Decleration to HMCTS West Yorkshire.

I've seen a couple of templates on here but it's not clear what i need to fill in and i'm getting stressed out with it all now!!

Do i fill in the top part on the template which says

"To: A Designated Officer of the Court
Name of Sentencing Magistrates Court
Address
Address
Postcode

BY RECORDED DELIVERY AND BY EMAIL

someone@hmcts.gsi.gov.uk

Court Ref: (if known)
Bailiff company Ref:

Or is that for information/reference purpose?

Can someone help me please, thanks

Leave the reference blank.
Run this Checklist. If no joy, then we'll fix it

So which parts do i fill in please?

Fill in the top part with the address you are sending it to.

Ignore the email address

Ignore the ref numbers.

Write what you want to declare.

Take it to a solicitors to be witnessed and signed.

Cheers for that, much more helpful,
also, at the bottom where it says signed i presume that's the solicitor/legal person rather than myself and do i need to sign it anywhere ?

You sign and date it, then the solicitor signs it when you swear it's true in front of him/her.
